Studies on the care management applications for the terminal cancer patients

Abstract
This study applied care management on the hospice service which targets terminal cancer patients. The purpose of this study is to apply and analyse the care management methods in the practice of hospice service; thus to discover appropriateness, problems, and meaning of the care management methods in the practice of hospice service. The subject of this study was a 42-year-old male with terminal cancer who had been suffering with nasopharyngeal cancer. The care management was practiced for about 4 months from intake to termination. This study utilized the care management process of Rothman for the analysis of the process. The result of the analysis confirmed that care management through a professional team conferred a positive impact on the quality of life for the client¡¯s residual life. The outcome of this study in the physical, psychological, social, economical and spiritual aspects is as follows. First, the positive change as a result of care management intervention in physical problems and the solution of desires was confirmed; the client became more comfortable thanks to the physical pain management and the alleviation of unpleasant symptoms. Second, care management intervention caused a positive impact in psychological and emotional aspects. The client was in denial and in anger during the initial period of intervention but he became more relieved psychologically and emotionally after the intervention. Third, the care management had a positive impact in social and economical aspects. Especially, the relationship with the client¡¯s spouse and children was improved and there were more conversation, interest and love between family members. Fourth, there was a tremendous positive impact in spiritual aspects. The client was at first constantly frustrated and annoyed due to anxiety and fear of death but he showed positive change by overcoming the fear, accepting death and pledging to donate his organs and preparing his death. Therefore, applying care management on the hospice service which is for terminal cancer patients can be a way to improved the quality of service. In addition, the care management which can effectively improve the quality of life of terminal cancer patients¡¯ residual life will require an interdisciplinary team approach.
